Continuance of Care

At Freedom Recovery Center, we're committed to providing high-quality care and support to individuals seeking treatment for addiction. We understand the importance of continued care in the recovery process, and we're here to partner with you in helping your patients achieve lasting recovery.

Our program offers comprehensive outpatient care, including monthly oral drug screens and appointments with one of our providers.

If you need to refer a patient for continued care, please complete the form below. Once we receive the completed form, our team will reach out to you to set up an appointment for the patient.

Emergency Patient Services

Patients may qualify for emergency addiction treatment services if they fall under the following categories:

Emergency Department visit within the last 48 hours: If a patient has recently visited the Emergency Department due to addiction-related issues, they may qualify for immediate treatment services.
Immediate Discharge from an in-patient addiction treatment facility: Patients who are discharged from an in-patient addiction treatment facility and require continued care may qualify for emergency services.
Referral from a community partner requesting Emergent Services: Community partners can refer patients who are in urgent need of addiction treatment services.
Immediate Release from a punitive center without an active treatment plan: Patients who are released from punitive centers without an active treatment plan in place may qualify for immediate assistance.

Patients who adhere to this contract will receive the following services at no charge for the duration of 60 days:

An office visit with a provider specializing in addiction medicine every two weeks: Patients will have regular visits with a specialized provider to monitor their progress and adjust treatment as needed.
Access to community service support: This includes counseling, job placement assistance, housing support, insurance enrollment assistance, and life skills support to help patients rebuild their lives.
Access to a certified Trauma/Recovery Coach: Patients will have access to a certified coach who can provide support and guidance throughout their recovery journey.
Drug Screens to monitor treatment progress: Regular drug screens will be conducted to monitor treatment progress and ensure patients are staying on track with their recovery goals.
Pharmaceutical support to ensure access to medication: Patients will receive support to ensure they have access to necessary medications as part of their treatment plan.
