Outpatient Addiction & Recovery Services Through Responsible Medication-Assisted Treatment

Continuance of care...
Need to refer a patient for continued care? Freedom Recovery Center's program offers outpatient care with monthly oral drug screens as well as appointments with one of our providers. To make a referral please complete the form below. Once we receive that completed form we will reach out to set up an appointment.
Emergency Patient Services
Patients may qualify for emergency addiction treatment services if they fall under the following categories.
-Emergency Department visit within the las 48 hours.
- Immediate Discharge from an in-patient addiction treatment facility.
-Referral from a community partner requesting Emergent Services.
-Immediate Release from a punitive center without an active treatment plan.
Patients that adhere to this contract will receive the following services at no charge for the duration of 60 days:
-An office visit with a provider that specializes in addiction medicine every two weeks.
- Access to community service support: counseling, job placement, housing, insurance enrollment, and life skills support.
-Access to a certified Trauma/Recovery Coach.
- Drug Screens to monitor treatment progress.
- Pharmaceutical support to ensure access to medication.
